At J+MED on Madison (Tomonori Nakagama MD, PLLC), we conduct medical examinations for Green Card (Form I-693) applications.

1. Identity Verification and Form Completion:
   Submit a valid passport, complete personal and medical information, and provide vaccination records.
2. General Examination:
   Doctor will perform basic physical exams and assess overall health.
3. Lab tests:
   including blood and urine, will screen for TB, STDs, and other diseases.
4. Vaccination Review and Supplementation:
   Doctor will check vaccines and give any required by immigration law.
5. Completion of Medical Examination:
   Doctor will complete Form I-693 and seal it in an envelope. Do not open.
